How to Use this Service:

To receive the names and telephone numbers of participating providers in your area, simply call the toll-free number indicated on your membership card or click here. Provide the customer service representative with your name and membership number. Then call the participating provider to make an appointment. Make sure to identify yourself as a UCA member.

Important: Show your UCA card to the provider’s assistant each time you visit the provider.

If you are currently working with a provider who is not in the network, you may provide the customer service representative with the provider's name, address and telephone number. We will be happy to invite the provider to apply for participation in the network. Unfortunately, The Alternative Care Program cannot guarantee acceptance by either the program administrator or the provider.

The Alternative Care program is NOT insurance; it is a member reduced-fee-for-service program that provides discounts at certain providers. In order to receive the special pricing, you must pay in full at the time of service.
